Hospitals and treatment centers often lack a reliable, accurate continuum of care once a patient is discharged. This gap leaves individuals vulnerable, unsupported, and at risk of relapse or overdose. Together We Grow, Inc. serves as the critical link between discharge and real, ongoing support. We ensure people receive accurate referrals and meaningful follow-up appointments with local providers who can help them rebuild what has been lost.

 Together We Grow, Inc. seeks grant support from the Substance Abuse and Mental Health Services Administration (SAMHSA).

Across the United States, the need for dependable addiction treatment referrals and behavioral health resources has never been greater. Communities require 24/7, accurate referral services that connect people to real programs—not tomorrow, not next week, but right now. Emergency rooms are overwhelmed with overdoses and individuals in crisis. Every day, medical teams are forced to make impossible decisions: who can be revived, and who cannot.
